A Clinical Study of Nonpenetratng Spleen Rupture


Abstract
This report represents a clinical study of experience with 26 cases of nonpenetrating injuries
of abdomen causing rupture of spleen, which were treated by surgical intervention at the Pusan
National University Hospital during 12 years from January-1958, to July-1970.
1. The sex distribution assumed a ratio of 3 to 2 with 15 males and 11 females.
2. The age distribution indicated a predominat incidence in 1st and 4th decade(38.4%).
3. As the causes, in order of frequence, traffic accident was in 19 cases(72.96%), violence in
4 cases(15.36%), falling in 3 cases(11.52%).
4. Among nonpenetrating spleen rupture in 26 cases the associated injury was in 12(46.08%),
rib fracture was the commonest incidence, involving 5(19.20%), colon rupture was 4(15.36%),
stomach rupture was 1 case and mesenteric injury was 2 cases.
5. The seasonal distribution indicated a predominant incidence in March and April involving
10(38.4%).
6. Physical findings and symptoms were variable, abdominal pain was in 57.6% and abdominal
rigidity in 46.08%.
7. On admission, C.B.C., especially, hematocrit and leucocyte count were elevated.
8. Abdominal tapping was valuable in diagnosus, positive findings were 65.4% in 26 cases.
9. Among 26 cases checking X-ray of chest and abdomen, the suggestive findings of spleen
rupture were 30.7%.
10. The single laceration of spleen was the commonest character of spleen rupture, 13(49%).
11. The splenectomy was done to all 26 cases.
12. The postoperative complication was 12 cases(46.08%) and the commonest one was adhesive
ileus, 15.38%.
13. Postoperative death was in 4 cases(15.4%), mortality was related to hemorrhage and
associated injuried organs.
